Required Yearly Forms

Important Yearly Forms to Return to School Nurse:

  • You will receive these the first day of school
  • Please fill out and return to nurse as soon as possible

1. Emergency Cards

Please complete both sides of the card and sign it. This information is used in the nurse's office to contact you if your child becomes ill or injured at school. The Parent or Guardian will be the first contact should a medical situation occur in the school. Any additional emergency contacts should be local and known to your child. If a life threatening illness or emergency occurs, your child will be transported by ambulance to the nearest hospital. Please contact the main office with any changes in phone numbers so that we can update your child's information.

2. Health Update Form

This annual update makes me aware of any current health concerns so that I can care for your child appropriately. Please complete in full.
